Work Packages

The project is organized into five Work Packages (WPs), each focusing on key areas that collectively ensure the comprehensive development and success of the CONNECT-CARE initiative. From overall management and coordination to technological development, stakeholder engagement, clinical evaluation, and results dissemination, each WP plays a fundamental role in achieving the project’s objectives.

Below is a summary of each Work Package, including its main objectives and the leading organization.

WP 1: Project Management and Coordination

Leader: Hospital Sant Joan de Déu

 Objectives:

The coordination of all administrative, financial, technical, and scientific aspects of the project is the focus of this work package. It will ensure timely execution, quality standards, risk management, and compliance with all ethical, legal, and data security requirements.

WP2: Co-Creation and Stakeholder Engagement

Leader: Fundacio Institut Universitari per a la recerca a l’Atencio Primaria de salut Jordi Gol i Gurina

 Objectives:

Designed to bring together a wide range of stakeholders, this work package promotes collaboration and cross-disciplinary dialogue through co-creation activities. It will develop both online and in-person spaces for shared learning, contribute to communication and outreach efforts, and support open innovation in key areas including obesity prevention, education, digital training tools, and ethics.

WP3: CONNECT-CARE Platform and NutriFit Advisor Development

Leader: ARTEMIS RESEARCH AND INNOVATION S.R.L

 Objectives:

Focusing on the development of digital tools to support healthier eating habits, this work package will begin by analyzing and organizing existing data so it can be used by a smart recommendation system. Expert knowledge in nutrition will also be added in a way the system can understand. Based on this, a mobile app will be created to offer personalized nutrition advice, combining artificial intelligence with expert input. An online educational platform will also be developed to help people improve their eating behaviors, and all of this will be integrated into the CONNECT-CARE platform.

WP4: Intervention and Evaluation: Pilot Studies

Leader: Region Hovedstaden

 Objectives:

This work package aims to assess the feasibility, effectiveness, and cost-effectiveness of CONNECT-CARE in two pediatric palliative care centers. A clinical trial will be designed, ethical approvals will be obtained, and staff will be trained in the use of the tools and the digital platform. Over a 12-month period, the intervention will be implemented and evaluated in real-world clinical settings, generating key data for its validation and future implementation.

WP5: Communication, Dissemination and Exploitation

Leader: Hospital Sant Joan De Déu

 Objectives:

To increase the visibility of CONNECT-CARE and promote uptake of its results, this work package will share project progress with key audiences such as healthcare professionals, policymakers, and researchers. A multi-channel communication plan will be implemented, strategic partnerships and commercialization opportunities will be pursued, and a collaborative network will be built. The impact of these activities will be regularly assessed to adapt strategies as needed.
